We understand that unplanned issues can come up and you may need to cancel or change an appointment. If that happens, we respectfully ask for scheduled appointments to be canceled at least 48 business hours in advance.
Our doctors and staff want to be available for your needs, and the needs of all of our patients. When a patient does not show for a scheduled appointment, another patient loses the opportunity to be seen. Although we have always had a cancellation policy, circumstances have caused us to enforce a policy of charging for NO SHOW appointments and cancellations with less than 48 business hours.
There will be a fee of $75.00 assessed to your account if a PHONE CALL to cancel an appointment is not received within 48 business hours of your appointment. In addition, if you cancel Doctor planned services within 48 hours of your scheduled appointment, you will be charged 15% of the service fees to be rendered.
Thank you for being a valued patient and for your understanding and cooperation. This policy will enable us to open otherwise unused appointments to better serve the needs of all patients.
We ask that you sign our Appointment Cancellation form and return to our office during your next visit.
Our office is committed to helping you maximize your insurance benefits. There are over 150 dental insurance companies, and policies vary for each employer. We can contact your insurance company and estimate your coverage in good faith, but cannot guarantee your exact amount of coverage due to the complexities of insurance contracts.
We are happy to request pre treatment estimates from your insurance company upon your request. Please remember, pre treatment estimates take 4 to 6 weeks to receive back. If you want treatment to be completed by a certain date, please schedule your dental examination early enough to allow time for your insurance estimate to be processed.
As a courtesy to our patients, we bill insurance companies for our services and apply insurance payments to account balances due. Your estimated patient portion must be paid at the time services are performed.
